Abstract Background Situs inversus totalis (SIT) is a condition in which the arrangement of internal organs mirror image normal anatomy, occurring 0.0001% patients. A thorough understanding human anatomy and anatomical variances crucial everyday clinical practise emergency surgical care. We present video on patient with SIT who underwent Laparoscopic Cholecystectomy (LC). Case Our 40-year-old lady known diagnosis SIT. She presented jaundice left upper quadrant pain (LUQ). An ultrasound CT scan confirmed gall stones, CBD stones an ERCP to clear stones. LC one commonest general procedures, however, this case proved challenging due reversed anatomy. Intraoperatively, operating positioning was imaging ports used normally. Standard 10mm were placed infraumbilical epigastric regions remaining LUQ. Abdominal inspected confirmed. It ensured that slow, meticulous delineation Calot's triangle window safety performed. The cystic duct artery clipped. rest procedure uneventful. Results discharged same day as procedure. There no immediate or short-term complications. Gallbladder histology showed chronic cholecystitis, cholelithiasis cholesterolosis.
